41 year old female with complaints of unable to talk and altered sensorium
Patient came with the complain of unable to speak and altered sensorium since three days patient was apparently a symptomatic three days backThen developed sudden onset of loss of speech and unable and altered sensoriumIt was observed by the attenderPatient apparently went to nearby diagnostic Centre to give give her a blood sample for sugar after coming back to home she had a sleep for two hours after which she was unable to speak and attender noticed acted sensorium and could not draped sari weakness of left arm and left lower limb was observed by the attender she was taken to nearby consultant where her BP was 190/90 DRBS3 300 for which she is given medication and suggested for MRI she is a known case of diabetes and hypertension for which she is using medication regularly general examination patient is conscious non-coherent noncooperative Vitals BP 1 3080 pulse rate 72 respiratory 16 cycles per minute respiratory system S1 S2 heard no murmurs of cardiovascular system S1 S2 heard no murmurs investigations
